PMT Assessment
This form can be used to record milestone achievements in babies and young children. It is suitable for use by therapists and health visitors as well as GP’s. It allows you to document the age at which an infant achieves a developmental milestone as well as identify if by not achieving a milestone this constitutes a red flag. It allows you to record several visits to document progress.
